What We Do

Memory and Aphasia Care provides Medicare-reimbursable outpatient services to support people living with cognitive challenges and their family members, through compassionate and person-centered care. We serve our families through virtual visits, home visits, and in our clinic locations.

Cognitive Assessment and Care Planning

We support patients and their families who are experiencing memory or thinking changes and are waiting months to see a neurologist. Our expert Nurse Practitioner can begin the evaluation process and can provide families with ongoing support and resources.
